Official Modified Electronic Class Record (ECR) and School Form (SF 10) for SSHS
The modified School Form 10 for the Strengthened SHS has three (3) sheets, consistent with the existing SF-10 for SHS. Each sheet contains the same sections as the current version; hence, the required information generally remains the same. However, certain sections include minor modifications that may require additional data inputs.

1. Is there a device or software requirement to use the modified SF 10 for SHS?
Since the electronic tool is in Microsoft Excel format, a computer with MS Excel installed is necessary. For smoother use of the tool, MS Office 365 (64-bit version) is recommended. Access to the licensed platform should be done using a DepEd account.
2. Why does the subject dropdown only show the option “List ACADEMIC/TECHPRO ELECTIVES in the ANNEX sheet”? How can this be resolved?
The dropdown option “List ACADEMIC/TECHPRO ELECTIVES in the ANNEX sheet” appears when:
• Academic and/or TechPro electives have not been listed in the ANNEX sheet; or
• The electives listed in the ANNEX sheet do not apply to the selected grade level.
To resolve this, proceed to the ANNEX sheet and list the appropriate electives. Ensure that the subjects listed apply to the learner’s grade level (Grade 11 or Grade 12).
3. Why do some subjects or electives not display semestral average grades?
Subjects whose final grades are computed over four (4) quarters, such as Core subjects, do not display semestral average grades.
Semestral averages appear only for subjects whose final grade is computed from two (2) quarters. The Remark column indicates the appropriate computation.
4. In the Summary of Final Grades, do teachers manually compute or re- enter the quarterly grades and recomputed grades from the Remediation Class?
No. There is no need to manually enter quarterly or remedial grades or to compute averages. The template automatically performs these calculations.
5. How can it be determined if a learner needs remediation?
The Summary of Final Grades displays the final grades for all subjects taken by the learner. If the General Average under the FINAL GRADE is below 75, the learner requires remediation.
If the recomputed grade from the REMEDIATION CLASS raises the general average to 75 or higher, the learner may meet the passing requirement.
6. When filling out the REMEDIATION CLASS section, where should the “Failed Final Grade” be obtained?
The Failed Final Grade may be obtained from the Summary of Final Grades, particularly for subjects whose final grades are computed over four (4) quarters. For subjects with semestral grades, the failed final grade may also be obtained from the corresponding semester table, where the grade is recorded.
7. Should SHS teachers who are not teaching Strengthened SHS subjects use the modified SF 10 for SSHS?
No. The modified SF 10 for SHS should only be used for classes under the Strengthened SHS curriculum. SHS Teachers handling subjects under the existing K-12 curriculum should continue using the current SF 10 for SHS issued through DO 69, s. 2016 >>.
