Basic Education Enrollment Form (BEEF)
New Students & Transferees: If you're entering Kindergarten, Grades 1, 7, or 11, or if you're transferring from another school, you'll need to fill out the Basic Education Enrollment Form (Enclosure No. 1). This form collects essential information about the student and their educational background.
Continuing Students: Students moving up to Grades 2 through 6, 8 through 10, or Grade 12 will confirm their enrollment using a simpler Confirmation Slip (Enclosure No. 2). This slip helps the school verify the student's intention to continue their education there.
Alternative Learning System (ALS) Enrollment:
All ALS Students: Everyone enrolling in the Alternative Learning System will use the Modified ALS Enrollment Form (Enclosure No. 3). This form is specifically designed for ALS learners and may differ from the standard enrollment form.
Required Documents and Deadlines:
Standard Requirements Still Apply:
The existing rules and requirements outlined in DepEd Order No. 03, s. 2018 (Basic Education Enrollment Policy) are still in effect. These requirements typically include documents like birth certificates, previous school records, and proof of residency.
Deadline Extension for Specific Documents:
While most documents need to be submitted promptly, there's an extended deadline of October 31, 2024, for providing a Philippine Statistics Authority (PSA) birth certificate.
Alternative Birth Certificate Options
If a PSA birth certificate isn't available, the following can be used as substitutes:
Birth Certificate (Late Registration): Obtained from the local civil registrar's office.
Barangay Certification: This certification must include the child's full name, parents' names, date of birth, and sex.
