Continuing students (Grades 2-6, 8-10, 12) confirm their enrollment using a Confirmation Slip (Enclosure No. 2).

New students and transferees (Kindergarten, Grades 1, 7, 11) need to fill out the Basic Education Enrollment Form (Enclosure No. 1).

Alternative Learning System (ALS) students use the Modified ALS Enrollment Form (Enclosure No. 3).

Required Documents:

Standard requirements outlined in DepEd Order No. 03, s. 2018 remain in effect.

If a PSA birth certificate is unavailable, a late registration birth certificate or Barangay Certification can be submitted until October 31, 2024.
