The Civil Service Commission (CSC) has also rolled out the updated Personal Data Sheet (PDS) for 2025.
Officially titled CSC Form No. 212, Revised 2025, this document is the primary requirement for anyone applying for a government position, as well as for current employees updating their 201 files or applying for promotions.
"Any misrepresentation made in the Personal Data Sheet and the Work Experience Sheet shall cause the filing of administrative/criminal case/s against the person concerned."
The new form (Annex H-1 of the 2025 Omnibus Rules) updates the previous 2017 version to align with modern standards.
• PhilSys Number (PSN): A dedicated field (Item No. 13) has been added for your National ID number, reflecting the government's push for the Philippine Identification System.
• Updated ID Requirements: The form aligns with current government identification systems for easier verification.
• Strengthened Data Privacy: The certification and warning sections have been updated to reinforce data privacy and accountability.
• No Blanks: Do not leave any field blank. If a question does not apply to you, strictly write "N/A".
• Name Format: Follow the format: Surname, First Name, Name Extension (Jr., Sr.), Middle Name.
• Dates: Use the numeric format dd/mm/yyyy (e.g., 16/01/2026) for January 16, 2016.
• Passport Photo: Attach a passport-size picture taken within the last 6 months. It must be a clear, front-facing photo with a white background and a handwritten name tag/signature.
What is the PDS?
The Personal Data Sheet is the official information sheet for all government officials and employees. How to Fill It Out (Quick Tips)
• Print Legibly: If writing by hand, use block CAPITAL LETTERS with a black or blue pen.• No Blanks: Do not leave any field blank. If a question does not apply to you, strictly write "N/A".
• Name Format: Follow the format: Surname, First Name, Name Extension (Jr., Sr.), Middle Name.
• Dates: Use the numeric format dd/mm/yyyy (e.g., 16/01/2026) for January 16, 2016.

The PDS is rarely submitted alone. You must almost always attach the Work Experience Sheet (WES) (Annex H-2), especially when applying for a position, to elaborate on your specific duties and functions in previous jobs.
