Are you looking for an updated, printable School Form 9 (SF9) Learner's Performance Report for Grades 4, 5, and 6 for School Year 2026-2027? According to the latest Department of Education directives, intermediate-grade levels are transitioning to updated classroom assessment standards.
Per DepEd Order No. 15, s. 2026, the Performance Report Template for Grades 1 to 12 shall be reproduced by the SDO or school, as applicable, for each enrolled learner. This template may also be used for Grades 2 and 3 during the SYs in which the numerical grading system is still being implemented.
