What Changed in the PDS Revised 2026?
The CSC previously released a 2025 version of the PDS alongside new HR rules, but a critical formatting error slipped through. The 2025 form accidentally deleted the columns for "Monthly Salary" and "Salary/Job/Pay Grade & Step Increment" in the Work Experience section.
To fix this, the CSC issued Memorandum Circular No. 05, s. 2026 in June 2026, officially mandating the Revised 2026 version.
- The Crucial Salary Fix: The 2026 form permanently restores the Salary and Pay Grade columns. If you are a teacher applying for ranking and promotion—such as aiming for Teacher I, stepping up to Teacher V, or applying for Master Teacher roles—accurately reflecting your exact Salary Grade and step increments from previous appointments is heavily scrutinized to determine your career progression and ranking points.

To ensure completeness of the PDS, the CSC hereby adopts CS Form No. 212, Revised 2026, which restores in the Work Experience section (Item No. 28, Section V.) the columns for: 1. Monthly Salary; and 2. Salary/Job/Pay Grade and Step Increment.
As a transitory measure, for a period of three (3) months from the effectivity of this Memorandum Circular, PDS accomplished using CS Form No. 212, Revised 2025 may still be accepted as an attachment to appointments submitted for attestation. Thereafter, CS Form No. 212, Revised 2026 shall effectively supersede the 2025 version attached to the 2025 ORAOHRA as Annex H-1 and shall be the only accepted form for such purpose.

5 Pro Tips for Filling Out the CSC Form 212
Because the PDS is a sworn legal document, any misrepresentation—even accidental—can lead to administrative charges. Follow these strict CSC guidelines to ensure your form is accepted without issues:
- Print Legibly in Block Letters: Always use capital letters. If you are typing it out using the Excel or fillable PDF versions, use a clean, standard font like Arial.
- The "N/A" Rule: Never leave a field blank. If a section or question does not apply to you, firmly write N/A. Do not use dashes or abbreviations.
