School Form 9 - SF9 (Learner's Progress Report Card) Templates

NEW! Prototype of Interim School Forms for SY 2020-2021 

The “Guidelines on the Checking and Preparation of the School Forms,” and DO number 58, series of 2017, known as “Adoption of New School Forms for Kindergarten, Senior High School, Alternative Learning System, Health and Nutrition and Standardization of Permanent Records” serves as guide in preparing School Forms.

The new, standardized forms shall be used in all public schools and ALS Community Learning Centers starting SY 2017-2018. Private schools, state and local colleges and universities as well as other higher educational institutions offering basic education are enjoyed to adopt the said forms.

Here are some of the important reminders in preparing SF 9 Learner's Progress Report Card for all Grade levels except for Kinder:

1. Rename the Form from Form 138 to SF 9 Learner's Progress Report Card (DO 58, 2017)
2. The format and content as stipulated in DO 8, 2015 (DO 58, 2017)
3. Learner’s Name should be written the way it was generated in the SF5 from LIS (DO 11, 2018)
