This form is an Excel-based version of the Part I-IV of the Individual Performance Commitment and Review Form (IPCRF) as prescribed in the DepEd Order No. 2, s. 2015.

It is intended to assist Teachers/Ratees in the preparation of their IPCRF forms at different RPMS phases, e.g. Individual Performance Commitment and Review Form (IPCRF) and Development Plan during Performance Planning and Commitment phase, accomplished IPCRF with computed final rating during Performance Review and Evaluation phase. 

The data gathered using this tool will help the Rateer in identifying the needs of teachers and in providing appropriate interventions.
