Simplified Guidelines in the Checking of School Forms

The goal is to ensure that everyone understands how to simplify and accurately check school forms, allowing all teachers to complete their tasks promptly and enjoy their vacation.
Only SF1, SF4, SF5, SF6 and SF 10 are subject for checking.
Ang pinakamahalaga sa cheching ay yung consistency at accuracy ng data.
Mayroon pang isang probisyon sa Deped Order 11,”It should be written the way it is generated in the LIS,” pero walang sinabi na porke’t naka ALL CAPS , isusulat na rin itong naka ALL CAPS. Ang probisyong ito ay patungkol lamang sa format ng learner’s name sa SF10 – yung last name, first name, extension name and then middle name; it is not referring to the font style or size but the arrangement of names pertaining to SF10.
“We also want to give emphasis na yung time po sa pagchecheck ng forms is only limited, limited din lang po yung scope ng tsinecheck – HINDI PO KASALI SI SF9 (Learners Report Card) SA TSINECHECK dahil hindi po primary document si SF9."
“Yung SF9 o Progress report ay ginagamit lang po if yung SF5 and SF10 ay hindi po magtali o magtugma.”
Istriko tayo sa pagcheck sa pangalan, date of birth at gender. May nakita kami pati pangalan ng nanay at tatay by spelling tsinecheck – hindi na siya kinakailangan, noh. Dun lang muna tayo sa mga data na mayron sa LIS – yun lang pangalan ng bata at mga detalye nito.
Ang SF5 at SF10 ay tsinetsek din yan pagdating sa GENERAL AVERAGE at end of school year status – promoted or retained. Pero kung hindi nagtutugma ang general average sa SF10 at status sa SF5 at SF4 dun lang natin kailangan ang SF9.
Again, inuulit natin – ang SF9 o progress report card, as per policy is not in the list ng mga major documents. Ang SF9 ay pangvalidate lang just in case na mayroong conflict and SF5 at SF10. Ibig sabihin nito, mababawasan na ng isang tao ang checking committee a division level na humahawak ng SF9.
This time, sinasabi ng policy, as early as 2018 – SF9 is no longer part of the major documents to be checked. Ginagamit lamang ito kapag nagkoconflict yung dalawang document na isinulat manually, which is the SF10 at itong SF5 na generated sa system.
Sana malinaw tayo dun para pati yung ibang mga question tungkol sa SF9 kagaya ng class days ay skip po natin yan sa checking, hindi po siya kasama sa scope. Isa yan sa mga adjustment – nasanay kasi tayo dati na parehong-pareho ang laman ng SF9 at SF10, babasahin natin ng sabay. But this time, babawasan natin yung mga forms na tsinecheck.
More reminders:
Only SF1, SF4, SF5, SF6 and SF 10 are subject for checking. SF2 and SF9 are just secondary report and need not be checked unless for validation purposes. SF 2 will be used only as reference in case of inconsistencies in SF1,SF4 & SF5.
SF9 can only be checked if SF 5 and SF 10 did not match. SF3 & SF8 are not supposed to be reviewed or checked by SCC and DCC.
PSA Birth Certificate is preferred but not required or mandatory.
Other equivalent document can be used as reference to establish identity of Learner
Focus on Accuracy and consistency rather than cosmetic and format.
